I am writing to inform you that I will be closing my vascular surgery practice at above location effective December 31, 2025. It has been a privilege to collaborate with you in the care of our shared patients, and I am grateful for your trust and partnership over the years.
To minimize disruption in patient care, I encourage you to refer patients who may require ongoing vascular evaluation or treatment to another vascular specialist; for example, Sutter Medical Group vascular specialists (Sacramento and Roseville) or within Hills network including UCDavis vascular surgeon or independent vascular specialist (Monahan vein clinic at Roseville) of your preference. Patients may also reach out to their primary care providers for assistance in establishing new vascular care.
Medical records will remain secure and available upon patient request. Record transfer requests should be directed to my office before November 30, 2025 and it will take up to a month for record transfer. After December 31, records will be available by Sutter community connect EPIC Practice Exit Records or separate storage service and please contact at vascularsurgeonlim@gmail.com
